Leviton Flush Mount
Receptacle Safety Notice
LEVITON MFG. has issued a safety warning on certain
flush mount receptacles manufactured between June 1 and
November 15, 2005. The receptacles in question (catalog
numbers: 278, 279, 5206, 5207, 5371, 5372, 5373, 5374, 7313,
9730 and 9750) may fail during installation or while in use and
present a risk of serious shock.
Date codes of the potentially defective items, which can be
found on the LEVITON label on the outer master carton, have
six characters that begin with:
If the master carton is not available, the code on the
mounting yoke (strap) will identify the device. The yoke code
will have either four or six characters in it. If it is the six
character format, use the above date code listing. If it is the four
character format and there is a “5” in the fourth position, the
receptacle is potentially affected.
Receptacles with date codes matching this criteria should be
returned to your local Leviton distributor for replacement.
